Community Adult Mental Health Service

This will be achieved by:
- valuing individual lived experience as well as family and carer involvement
- providing a range of services when and where people need them, delivered by skilled, compassionate staff
This is a referral service. Referrals can be made by:
- your doctor
- another health and social care professional
What happens next?
After referral, we will work with you, your family and carers to assess your needs and wishes.
We will use this information to create your support plan. The plan will help us to support you towards goals and promote your recovery. You may then receive support from a service arranged by us.
Will I be charged?
There may be a charge for support services. Your care manager will discuss this with you.
You may be eligible for self-directed support. This gives you a range of options, including receiving a direct payment to enable you to buy support services of your choice.
Carers
If you care for someone with mental ill-health, you can complete an Adult Carer Support Plan or a Young Carers Statement. These give you the opportunity to identify your needs and any support you may require, to continue providing care.
Advocacy services
The support of an independent advocate can help you to express your opinions or wishes. We can provide information about advocacy services.
Helplines
You may find the following helplines useful:
Samaritans: a charity offering support to people who are suicidal or despairing, call 116 123 (lines open 24 hours)
Breathing Space: free confidential support for people experiencing low mood, depression or anxiety- call 0800 83 85 87 (Monday to Thursday, 6pm to 2am; Friday 6pm to Monday 6am).
Preventing suicide
If you are worried that someone is suicidal, please ask them. It could save their life.
We work to prevent suicide through the Choose Life initiative. Our Choose Life co-ordinator works with you, communities and organisations to raise awareness of suicide.
We help you to recognise the signs of someone in distress via training courses such as Safe Talk and Applied Suicide Intervention Skills (ASIST). This gives you the confidence to intervene.
